Abdominal and Pelvic Pain During Pregnancy

If you experience unexpected abdominal and pelvic pain during pregnancy, it's easy to worry. But while some pain may need medical attention, sometimes instances of pain are relatively harmless.

Some Reasons for Abdominal Pain

As you go through pregnancy, you may find that you experience various pains in the abdominal area; these are generally normal although they can be very painful or uncomfortable. Here are some of the reasons you may experience abdominal pain:

1. Early Pregnancy - When the Embryo Starts to Embed itself

Discomfort in the abdominal area is common during Early Pregnancy. One of the first times when you may experience pain is around the time of your first missed period. For a day or two, some women have a pain that's low down in the abdomen and feels a bit like a dull period pain, as if your period is about to start. It's thought that this is because the embryo is beginning to embed itself in the soft lining of your uterus at this time.

2. The 3rd/4th Month - Ligament Aches

During the third or fourth month of pregnancy, women frequently get a sharp stabbing pain in their groin or to one side. It may come on at sudden intervals, might be worse if you suddenly get up, stretch or move and it might disappear then reappear a few days later. It can be really worrying to suddenly get pains like this, but usually it's due to the ligaments or muscles around your uterus in your pelvis stretch and thicken as they support your growing baby.

3. When the Baby Grows - Constipation and Bloating

Constipation, bloating and wind can also cause aches, pains and abdominal discomfort. Constipation is often caused by the pressure of your growing bump on your rectum, as well as hormones which are said to slow down the movement of food through your digestive system. These factors are also involved in bloating and wind, too, with your growing uterus pressing on your intestines.

4. Later Pregnancy - Braxton Hicks

In later pregnancy, Braxton Hicks contractions may cause some pain. These contractions, which are irregular tightenings of the uterus, can occur from early pregnancy, but most women don't notice them until the latter half. They're generally started of by either you or the baby moving, or if someone touches your stomach. It's a normal part of pregnancy, but can feel very odd. If you're concerned or the contractions become more frequent, talk to your doctor.

Abdominal Pain to Worry About

During early pregnancy it is always advisable to seek medical advice with any abdominal pain. The first thing the doctors/midwife will check for is ectopic pregnancy.

Ectopic Pregnancy

Ectopic pregnancy happens when the fertilised egg implants itself outside of the womb. The most common place for this to occur is in the fallopian tube, this is known as a tubal pregnancy. Ectopic pregnancies can also occur in an ovary, the abdominal space and also in the cervix (which is the neck of the womb).

Here are a few things to note about ectopic pregnancies:

  • Ectopic pregnancies are serious and because of where the egg has implanted itself in the fallopian tube it can not be saved
  • With ectopic pregnancies early diagnosis is key, if diagnosed early the pregnancy can be ended safely and without any complication
  • Ending the pregnancy would be done using medication or surgery depending how far along the pregnancy is
  • Early diagnosis also greatly improves chances of a normal pregnancy at a later date. If it is not diagnosed and is left untreated a tubal pregnancy can be fatal, the fallopian tube can split causing internal bleeding and blood loss

  • In the UK 1 in 80 pregnancies is ectopic
  • There are at least 11,000 ectopic pregnancies in the UK each year
  • In the UK 5 (approx) women die each year from ectopic pregnancy.

Abdominal Pains: When to Contact a Medical Professional

If you're at all worried about the abdominal pain you're getting, always consult a doctor, midwife or maternity unit immediately. This is particularly so if you experience any of the following symptoms:

  • During early pregnancy – severe cramp in your lower abdomen, which lasts for several hours, is accompanied by vaginal bleeding and doesn't ease. This could be the signs of a Miscarriage.
  • During early pregnancy – severe pain on one side of your lower abdomen, plus bleeding or a brown discharge from your vagina. This could be sign of an ectopic pregnancy.
  • At any time – pain or discomfort when urinating, a fever, pelvic discomfort, a frequent urge to urinate, chills, pain in your lower back or side, nausea, vomiting or blood in your urine. This could be a sign of a Urinary Tract or kidney infection, which being pregnant makes you more susceptible to.
  • During later pregnancy – pain in the upper abdomen, along with symptoms such as a very bad headache, nausea, vomiting, flashing lights or seeing spots in front of your eyes. This could be a sign of Pre-eclampsia.
  • During later pregnancy – severe pain in your lower abdomen, with or without bleeding. This could indicate a separating placenta.

Pelvic Pain

Occasional aches in your pelvic region are common and usually nothing to worry about, but one in 35 women get pelvic joint pain (Symphysis Publis Dysfunction). During pregnancy, a hormone called relaxin is released, which softens the joints ready for the birth of your baby. But for women with pelvic joint pain, the hormone goes overboard and causes the ligaments to soften and stretch too much.

The pelvis is made up of four separate bones which are joined at the front by the symphysis pubis and it's normal for there to be a gap of 4-5mm between the two pubic points. During pregnancy, the gap widens by another 2-3mm, but when it stretches too far it results in pain and discomfort. This normally occurs from the second trimester onwards, but it can happen at any stage or even after you've given birth.

The Symptoms for SPD Include:

  • Pain in your pubic area and groin.
  • Pain in the inside of your thighs.
  • Lower back and hip pain.
  • Pain when you walk, move or go up stairs.
  • A clicking sound that you can hear.
  • A feeling that your bones are grinding together.
  • Difficulty and pain when you try to open your legs.

If you experience any of these symptoms, then speak to your doctor or midwife. If they do think it's SPD, you'll be referred to an obstetric physiotherapist, who can offer the best course of treatment. It can make giving birth a bit uncomfortable, but make sure everyone knows what's wrong and that you find a position that is comfortable for you.

Hi wonder if you could help I wet to epu and was told they couldn't find a heartbeat I started heavily bleading and been told to wait till Friday to have another scan which is 8 days well the last 2 days I've had real bad pain in my groun eria that keeps coming and going real strong an din my back I'm only 8 weeks so deffo not labour but the pain is still there I'm worried it's something not related to the pregnancy and I'm scared please help!!

Hi im 31 weeks pregant with my first. Within the last few weeks i have been experiencing sharp pains in my pelvic area along with a constant pain in my groin whenever i move my leg up or to the side. I have also started getting cramps in my lower back and in my abdomen and it feels like im going to start my period. On top of that when i walk for a long period of time my left side starts to cramp up and it hurts so bad i have to stop moving until it goes away. Im not sure if i should be worried or not. I have not talled to my obgyn yet because my next appointment isnt till Wednesday. I need some advice.

I was on ivf treatment last Dec through to Jan 2017....I stopped the treatment as I had a slight over hyperstimulated ovaries...
Since then I had a cycle on the 14th January that was ok....not sore or heavy...
Since then I have had no cycle but slight blood when I wipe....this was when I was due...last month
Since then i have a really hot lower back pain...pains in my front passage running to toilet...pressure on my bladder...hot flushes and fatigue...
I have been in time gynae and had my womb checked and all looked ok...they said I had a small cyst on my right ovary...but they did a ultrasound scan and in my uterus it showed a black peanut sized small black dot in the middle...they were baffled and asked did I do a pregnancy test I said I had done loads and came back negative...they made me wait for the resilt and it came back negative....has anyone had these symptoms and been pregnant and it not show on the blood test...
Losing my mind....any advice of similar problems would get greatly appreciated

What does your midwife say? We're guessing you're now on weekly visits to her/him by now? If it reassures you, you should ask for a second opinion and as it's your 4th pregnancy, they should not dismiss your worries - or should at least address the pain issue for you.

Im due march 5th but been having serve pain when laying down and getting up limp most of the time and if i sit on the floor it hurts like a sob to get up. Ive talked to my doctor and seems nothings wrong its my 4th pregnancy and this pain started 2 montha ago. To turn around on my other side when sleeping hurts to turn over the pain is horrible

Can anyone help im 30 weeks pregnant and the past 5 days ive been up all night feeling the need to urinate and it will only come out in drips and drabs ... im thinking it could be a UTI but im not feeling any pain what so ever im just constantly uncomfortable at night... also when im walking longer than a couple of minuites im getting stabbing pains in my pelvis area mainly more to my right... im just so confuse and its effecting me.. can anyone help?

16 months?! We're not sure how far through her pregnancy she is, but after 6 or 7 months, it is quite common ot feel pressure in the pelvic area. There could be other reasons for the pain so it's always worth getting her GP or midwife to check it out.

Hi I'm 4 weeks pregnant, I got pregnant again 2 weeks after having a miscarriage. I have got a dull ache in the bottom of my tummy in the middle, it's not always there and aches more when I lye down. It's not the same feeling as when I have miscarried (twice) and I have no bleeding. Do you have any idea what this could be? Hi im 30 week and 6 days im having really bad pain in my abdomain and it also runs down both sides of my tummy and my hips two i have had diariaha for 2 days now i am worried as my son was born 36 week n 6 days due to an abruption my placenta detacting before my son was born and i have been in and out of hospital with problems all threw this pregnancy and i was wondering what advice anyone could suggest as the pain is unberable and im also anemic and strugglin to sleep with the pain im in x

Hi, i am 18 weeks pregnant and am getting quite painful sharp pains to the left hand side of where my womb has grown. The rest of it just aches a little. Had a scan earlier today to measure cervical length (which is fine). This is the 5th pregnancy that i have had that has got to this stage, yet this pain is more frequent than i recall for any other pregnancy. Would this still be round ligament pain?

She should ask her midwife about this. It might be Braxton Hicks which are contractions caused by the tightening of the muscles of the uterus in preparation for labour. These are usually painless however. It may be the onset of labour and she's feeling actual contractions. The midwife will be able to reassure her.

Hello. I am about 14 weeks pregnant and I have jus recently had a UTI infection and are on antibiotics, plus I take a multi vitamin for pregnany, calcium, folic acid and eat a lot of yoghurt and eat regularly and keep to a balanced diet. I have abit of stress, but very slight. I have been experiencing sharp like pains, but very v very mild. What is the cause of this and is it normal? I rub my tummy all the time to soothe the my tummy and make my baby feel relaxed and comfortable. Should I worry? I'm planning to go for a scan in 2 weeks. What are your suggestions and advice? @kierra2205. I had these in early and late pregnancy and it did seem to be just a part of the body changes as there was nothing untoward. What is your midwife like? Ours was brilliant, you could just ring and ask her questions. If yours is the same, she may invite you into your local health practice simply to listen to the heartbeat and put your mind at rest.

I'm 11 weeks and 3 days pregnant I've got all symptoms of being pregnant especially being sick morning and night! My boobs have also grown exceptionally quickly and I'm already showing a little bump, I have previously been having a dull ache in my lower abdominal area but I am still being sick, I've been told by my family nurse it's where my womb is stretching, has anyone else suffered with this? I have also had a miscarriage a few years ago and I'm worried this could be happening again, I've already had a scan and everything looked fine and I saw the heart beat, I'm just worried something is going wrong but I have had no bleeding, could anyone help with this dull ache?

Me and my partner are trying for a baby, recently we did a test and it came up positive, however today my partner was feeling a sharp on her left side abdominal area she described it as feeling like something bit her really hard, she is experiencing cramp and the area is very tender to the touch I'm worried it could be Ectopic, could really do with help and advice.

@Puleng. As our article says, there are many reasons for abdominal pain whilst you are pregnant. If your doctor says it's fine then it probably is, but if it's so severe that you are worried then why not ask for a second opinion.

My last period was on the 31st of January. I missed my period in February. I did a pregnancy test using the Pregnancy Test Strip and it proved positive. I started spotting without pain two weeks after. It stopped five days later. I did the test again two weeks later and it proved Negative. Did an xray it was still the same. I started spotting two weeks after which last for five days but without pain. In May i had slight pain and a menstruating bleeding though the blood was thin. Twenty eight days after i had another Menstruation. Is it possible that i had had an abortion? i still trust God that it is not. Please help

I'm 11 weeks pregnant, it's my first baby and I'm having pain in my groin first it start from right groin then next will be my left groin, I also have back aches that comes and goes. I work as a carer and is been doing sometimes heavy lifting, hoisting, pushing and pulling at work. I'm doing 12 hours shift and 3days in a week sometimes 4days in a week. @babynaa. What checks were made and were you offered a scan to reassure you? Tell your midwife about your concerns at your next check up, which should be every month from now on.

I am now in my 17th week of pregnancy and i have been having really persistent abdominal pains since my 11th week. There's no spotting or bloody discharge. I spoke to my doctor and he claims there's no cause for alarm. I am very worried because i am having intermittent headaches that stay as long as 4-6 hours a day as well. Should i be alarmed??!

i am always having a severe pains in my lower back and in my abdomen since almost 4weeks of my pregnancy.i am only a month and a couple of days pregnant .i got worried. because seems this is not normal for me..what should i do?

My first missed period was 15th February I had some light spotting at the beginning of February I have done several tests and been to the clinic for a test they all came back negative or inconclusive, I have started taking folic acid but I have been experiencing these symptoms for weeks now surely I should have had a positive result by now? I have cramping but mainly in the morning, today I am due my next period and am experiencing pain again but it is more discomfort

I am 27 weeks pregnant and experiencing severe pains around my joints in my shoulders, wrists and fingers. It seems to go from one joint to another symmetrically
and lasts about a day and night in each joint. There is no swelling just pain and aching. Some days I can't grip or even dress. I am taking paracetamol and a low dose of Codeine as prescribed by my Dr but this is not helping. I also have a 18 month old and had no problems in previous pregnancy. Please help !!

i am approximately 3 weeks pregnant. i am having sharp pains in my lower abdomen no bleeding but its starting to worry me. the doctor says she cant tell if its serious or not because its to early for an ultrasound that will show anything. should i be worried? its my first pregnancy.

iam 32 weeks pregnant and iam haveing severe pains in my lower abdomen i do surfer from back pain and pain at the top off my tummy.however it has been a couple off days now and am getting a bit worried!!! Can anyone give me some information on this.

I took a home pregnancy test yesterday and it was positive. I havent been to the doctor yet but am planning to make an appointment first thing Monday morning. So far I have been having slight nausea, headaches, some mood changes, breast swelling and soreness, bloating, and some slight discomfort in my lower pelvic region right side. Are these changes normal during early (five weeks) pregnancy?

Hi girls..I've experienced pain as well when I was 6-7 weeks. I advise my Doctor, she said that this might be a threatened abortion. Good thing there was no blood, only slight pain I felt mostly in the evening. She recommended me to take Duphaston and Duvadillan. Which became very helpful on my pregnancy. I am not 12 weeks pregnant, my baby has a heartbeat already. I advise you, that everything you feel or any discomfort you should consult your doctor. They know best than us. Don't even try taking other OTC medicine which are not recommended for preggies. Goodluck to us

hi, i'm 6/5 Months Pregnant and have started experiencing pelvic pains on my left side, it's a sharp mild pain that comes and goes. It has been happening for a day now. should i get worried??? i had an ectopic pregnancy in January this year.

My Mother is a nurse by profession and she has advised that it mite be the baby trying to turn causing my body to stretch too much.

I just got off of Birth control about a month and a half ago, however I still had my period for June. Here It is July and now my period is late, What's really going on??? I have been having dull lower abdomen pains almost every night, pressure in the rectum occassionaly, and I took a pregnancy test that came out Negative... I need answers before I say a word to my mother, SOMEONE HELP!!!!

I have had some cramps followed by spotting i have spots around my nipples but all the tests i have done came back negitive. But this month when i had my monthly cycle the pain i got in my lower abdomen was so bad that it sreed to the top of my legs does this sound like it could be a miscarriage or an ectopic pregnancy

I'm now 7 weeks. I didn't find out till last week when I went to the hospital for a pain in my left abdomen and back. it turned out just to be a UTI. But now i'm having a dull pain on my right side that won't go away. and i'm not sure what this is cause I just got my appendix removed in march.

I am 28 weeks pregnant with a little boy. I notice I have braxton hicks quiet often. Its a really tight pressure tightening in your lower stomach. They dont hurt, but you definitely do feel then when they occurr. Its amazing to me because all I think about is my little boy and how much I cant wait for him to be here. Pains always since 2yrs ago. The test read pelvic infection another read stafiloccocus nd it causes miscarriage. Doctors av tried but no solution.

Tam; I sugguest you see your gp or midwife. I had pain in my left side around 5-7 weeks and it was nothing but you need to be sure. I am now 30 weeks and think Im having braxton hicks. Lots of very low pain in my groin like mild period paid. I was born 7 weeks early and my little girl was 4 weeks early...

I am about 5 weeks pegnant and have pcos and am on metformin. I just want to know if its normal 2 feel cramping on left side of ovary area as well as lower backache which is quite a dull pain and feeling pressure on my rectum. The pain is not severe and no blood discharge. Is this all normal or should I panic?

i suggest you go see your doctor cause its better to have your mind put at rest.am in my 24th week with my first baby and having pelvic and hip pains and i have an appointment tomorrow, from the syptoms i guess its pgp, had a miscarriage some years back and baby was just 6 weeks the abdominal pains were sharp and unbearable so i tend to take any abdominal pain during pregnancy serious.

I too have had the same thing, its a lower tummy pain near pelvic area comes and goes. Also had mini cramps in what feels like my cervix since conception. Today (3 days early) I am showing a strong positive test although been showing positive for around 4 days now so must be around 4 weeksish. Past miscarriages started with period type pains showing brown discharge that worsened rapidly in to pain and a bleed, this time however these pains feel more like the stretching pains/mild period cramps. If in doubt go straight to the Dr i did with my past experiences and the presnent one, get an early scan done its worth putting your mind at rest :-) good luck x

I'm having these same symptoms. No bleeding just constant pelvic pain for a few hours just started today and I'm about 6 weeks pregnant. Don't know if it's normal.

I am having severe pains in my lower abdomen and pubic area but no bleeding and it has lasted more than 2 days now. I am only about a month into my pregnancy. I haven't had any bleeding that i'm aware of but I'm not sure what's going on. It doesn't feel normal.
